a. Before participating in a school-sponsored sport season for grades 6-12, all student-athletes must have the following on file with the school:

i.  Grades 6-8: Any student playing a school-sponsored team needs to have a physical on file. The physical is good for all of middle school.

1. Athletic Emergency Contact Form
2. Consent for Athletic Training Services
3. Athletic Handbook Acknowledgment 
4. Electronic signature of the athletic code meeting clearance.
5. Sudden Cardiac Arrest Acknowledgement

ii.  Grades 9-12:

1. WIAA eligibility form
2. Athletic Emergency Contact Form
3. Consent for Athletic Training Services
4. Athletic Handbook Acknowledgment 
5. Electronic signature of the athletic code meeting clearance.
6. Signed physician physical in grades 9 and 11 and an alternate year card the other years
7. Sudden Cardiac Arrest Acknowledgement

b. Once you complete these steps, you are registered and can participate in Sevastopol middle school and high school athletics. Any questions should be direct to Brooke Tanck-Athletic Director, Sevastopol School District.
